Medication Error Definition
| A medication
error is any preventable medication related event occurring as a
result of actions by a healthcare professional that may cause or
lead to patient harm while the patient is in the care of the
healthcare provider (National Coordinating Council for
Medication Error Reporting and Prevention (NCCMERP, 2000).

| Many hospitals define
medication errors as those incidents when medications are:
(1) omitted,
(2) given at the wrong time,
(3) given to the wrong patient,
(4) the wrong dose,
(5) the wrong medication,
(6) the result of a transcription error,
(7) given to a patient with a known allergy,
(8) repeated without an order,
(9) given by the wrong route, and
(10) discontinued without an order.
However criteria for what represents a medication error
differ among institutions.
